The eyes are caused by rubbing/hitting against dirty tank walls. Not a major issue. Will recover with 3 weeks but you'll need to make it a habit to wipe clean the tank walls.
the main concern is why is it constantly hitting/dashing around in the first place ?
water no good?
sick?
jittery?
like what many pointed out, try to find out more about your water conditions.
That will solve the root of your problem.
