Suspend feeding & increase wc frequency

I did water change every 5-7 days. I have recently done 2 water change within 3 days after the incident.

Bro same fish, today I found his tail broke. Last change of water 2 days ago.

Then I found this a few days back in the tank, it's soft and it's a tissue membrane. I don't know where it came from, it stinks so bad.

Stop using active carbon and coral chips if you are.

Frequent wc better than carbon.

Shells of oyster and any type clam better as pH stabiliser.

I don't use any of those, those rock like pebbles u see at the bottom of the tank are de-nitrates.
