LPA and AMD any overlapping?

Anyone here done a LPA already? Free application fees $75 for Form 1 till Aug 2018.

do we need a lawyer to do the LPA or this fee of $75 is now waive and just get and send application to ?

We still need a lawyer or a doctor to issue a certificate and that costs much more than the application fee of $75 which is waived now till August 2018. Some docs charged as much as $200! They have a list in the site of those docs that do. Sadly polyclinic docs cannot do.

No need to pay too much as the process is the same.
The cheapest rate is at an old clinic below a Whampoa HDB block
Must bring along the application form.
The patient must be present and on a 1-1 session with the doctor after some verifications done.

Actually this LPA is to do what.....should be different from WILL and whatever nomination we made.

i had done the LPA for my mum. Only need to go to appointed panel of doctors to sign the forms, and pay them. need not go lawyer.
